Office Policies

HOURS AND APPOINTMENTS:

Our office hours are Monday through Friday from 8:00 am to 4:30 pm.  Our schedule is by appointments only.  We accept same day appointments.

If you are unable to keep your appointment please let us know as soon as possible, in order for us to make that time available for another patient.  We do not charge for cancellation of appointments; however, we encourage you to give us advance notice as courtesy.

PAYMENT

We will expect payment at the time of service, this includes co-pay that is part of your insurance contract and designated by your insurance plan.  If you have not satisfied your deductible at the time of your service, you will be expected to pay at the time of your visit.  As a convenience for you, we accept cash, check, MasterCard, Visa, Discover, American Express, or Debit card as a payment option.

We give 30% discount to patients with no insurance. 

INSURANCE

Our office will verify benefits and file your claim.  Please present your most current insurance card at the time of your visit.

NOTIFICATION OF RESULTS:

For normal results a card will be mailed to your home address.  We will contact you personally for abnormal results.  If we are not successful in contacting you via telephone, we will send you a certified letter to your home address.  We will not give any health information to anyone that is not authorized by you to obtain results.

AFTER HOURS/ EMERGENCY:

Emergency calls are accepted at all hours.  The doctor on call can be reached by calling our office (770) 427-2533 and leaving a message with your call back number.  The physician on call will return your call within 20 minutes.

TELEPHONE MESSAGES:

All calls will be returned within 24 hours. 

PRESCRIPTION REFILL:

In order to expedite your request, please have your pharmacy contact us for refill.  Allow for 24 hours.  You may also contact our office, please provide us with your pharmacy telephone number at the time of your request.  All requests may take 24 to 48 hours.
